x86-64: enable hypervisor output on VESA frame buffer
This is x86-64 only for now due to the virtual address space
constraints on x86-32.
New options 'vesa-ram', 'vesa-map', 'vesa-mtrr' are close equivalents
to the vesafb Linux options 'vtotal', 'vremap', 'mtrr'.
Also the font can be specified: font=8x{8,14,16}.
