Linux uses sse4_1 and sse4_2, but at the moment libxl uses '.' instead
of '_'. This makes it confusing for people looking in Linux's /proc/cpuinfo
to disable features.
Add the Linux feature names, keeping the old ones for compatability.

libxl_cpuid_destroy has been renamed to libxl_cpuid_dispose; also cpuid
functions are only available on x86, so move them to libxl_cpuid.c.

Move a lot of
#include <stdfoo.h>
from individual files into libxl_internal.h. This helps avoid
portability mistakes where necessary system headers are omitted from
individual files, and is also of course a convenience when developing.
Also add
#include "libxl_osdeps.h" /* must come before any other headers */
to the top of most libxl*.c files, so that anyone who adds any headers
before libxl_internal.h will put the in the right place.

libxl_internal.h now #includes libxl.h and various system headers.
This
1. makes the order of header inclusion more predictable
2. explicitly allows libxl_internal.h to use objects defined in libxl.h
3. removes the need for individual files to include these headers
Also
- remove some unnecessary #includes of libxl_utils.h,
flexarray.h, etc. in some libxl*.c files,
- include libxl_osdeps.h at the top of libxl_internal.h
- add missing includes of libxl_osdeps.h to a couple of files
- change libxl.h to libxl_internal.h in a couple of files

Allow to enable/disable SVM specific cpuid bits in the guest config
file via cpuid config option. Also allow to enable/disable the
hypervisor cpuid bit in the guest config file. We need to disable the
hypervisor cpuid bit to get Hyper-V going.

Destroy is an overloaded term which would commonly like to be used for actual
destructive operations, such as destroying a domain etc.
Dispose isn't a great term but it does the job.

I ran the following and committed the result. ^I is an actual hard tab
for i in $(grep -l --exclude=*_[ly].\[ch\] '^I' tools/libxl/*.[ch]) ; do
cat $i | expand | sponge $i
done
There are some actually wrong indentations too, I'll fix those up
manually.

there are some new CPUID features that are safe for guests to see, as
they don't require OS awareness (FPU/ALU related instructions only).
Among features for new AMD CPUs there is also the PCLMULQDQ bit, which
Intel CPU have already for quite a while.

separate file, with stubs for ia64.
